Even the most elegant technical architecture and enlightened engineering culture will be stifled by a misaligned organizational structure. Many digital transformations fail not because of flawed strategy or technology, but because the organization's structure, incentives, and power dynamics cannot support the new way of working. This article offers a reflective framework for reshaping our teams and culture to capitalize on our investment in observability and drive lasting transformation.
The Flaw in Our Design: Optimizing the Pieces, Sub-optimizing the Whole
Our organizations are logically structured around functional expertise: R&D IT, Manufacturing IT, Commercial IT, and Supply Chain IT. This design is excellent for developing deep domain knowledge. However, it is fundamentally misaligned with the flow of value to the patient. A CAR-T therapy journey, for example, cuts horizontally across every one of these vertical silos, from the commercial patient onboarding system to the manufacturing scheduling system, and finally to the supply chain logistics platform.
When each silo has its own budget, its own priorities, and its own metrics for success (e.g., MES uptime, CRM feature delivery), they will inevitably optimize for their local domain. This local optimization creates friction, delays, and communication gaps at the handoff points between silos—and it is at these seams where value is lost and risk is introduced. A feature released quickly by the Commercial IT team might inadvertently create a data format that the Manufacturing system cannot process, leading to manual rework and delays. 1. Charter a Pathfinder Team for a Single Value Stream Instead of trying to transform the entire organization at once, select one high-value, high-pain value stream. A perfect candidate is the "vein-to-vein" journey for a personalized medicine. You then charter a dedicated, cross-functional Value Stream Reliability Team (VSRT). This team is not a temporary project group; it is a permanent product team whose product is the reliable and efficient flow of that business process.

It includes not just IT engineers from manufacturing, supply chain, and commercial, but also a dedicated Product Owner from the business who lives and breathes the process, a data analyst who can translate telemetry into insights, and a partner from the Quality unit who acts as an enabler of compliant innovation, not just a gatekeeper.
